This form provides basic information about each of the spouses and any children involved. It also provides the firm with some initial information about your marriage and your goals and interests.

Financial Information Form
This form details the living expenses for your household, and assists you in determining what if any any additional support may be needed from your spouse.

If you and your spouse are living apart, please complete a separate form for each residence and include as much information as you have about each.

Marital Property Questionnaire
This form details the assets and debts for you and your spouse. It is lengthy and may take some time to complete. It's length is designed to give you an opportunity to explore all possible assets and debts that may be involved.

Providing us the information in this format also assists in keeping your fees lower because our familiarity with this form saves us time.
